ABSTRACT

Objective:To investigate the clinical application value of plasma SEPT9 gene methylation combined with serum carcinoembryonic antigen (CEA) and carbohydrate antigen 724 (CA724) in the diagnosis of colorectal cancer.Methods:A total of 219 patients with colorectal diseases in Baoji Central Hospital and Yunnan Province New Kun Hua Hospital from May 2018 to October 2021 were selected, including 149 cases of colorectal cancer and 70 cases of colorectal polyp diagnosed by pathology. A total of 100 healthy people in the same period were selected as the healthy control group. The methylation of SEPT9 gene in plasma was measured by using real-time fluorescent polymerase chain reaction (PCR), and the levels of serum CEA and CA724 were measured by using electrochemiluminescence. The expressions of three indicators in each group were compared, and the effect of every single indicator and the combination of the three indicators on the diagnosis of colorectal cancer was analyzed by using receiver operating characteristic (ROC) curve.Results:The positive rate of SEPT9 gene methylation in colorectal cancer group (74.50%, 111/149) was higher than that in colorectal polyp group (22.86%, 16/70) and healthy control group (1.00%, 1/100), and the difference was statistically significant ( P < 0.001). The positive rate of CEA in colorectal cancer group (46.98%, 70/149) was higher than that in colorectal polyp group (40.00%, 28/70) and the healthy control group (3.00%, 3/100) and the difference was statistically significant ( P < 0.001). The positive rate of CA724 in colorectal cancer group (38.93%, 58/149) was higher than that in colorectal polyp group (32.86%, 23/70) and the healthy control group (2.00%, 2/100), and the difference was statistically significant ( P < 0.001). The area under the curve (AUC) of ROC of SEPT9 gene methylation, CEA and CA724 in the single diagnosis of colorectal cancer was 0.823 (95% CI 0.753-0.891), 0.788 (95% CI 0.725-0.852) and 0.689 (95% CI 0.624-0.754), respectively. The optimal cut-off Ct value of SEPT9 gene methylation in the diagnosis of colorectal cancer was 36.5, the sensitivity was 90.30%, and the positive predictive value was 84.68%, which were higher than those of CEA and CA724. The optimal cut-off value of CEA in the diagnosis of colorectal cancer was 8.80 ng/ml, and the specificity (77.50%) and negative predictive value (78.48%) were higher than those of SEPT9 gene methylation and CA724. The sensitivity (97.66%), positive predictive value (93.98%), negative predictive value (81.25%) and AUC (0.846, 95% CI 0.749-0.944) of the combined detection of the three indexes taking the optimal cut-off value of every single indicator were higher than those of the single indicator. Conclusions:The combined detection of plasma SEPT9 gene methylation, CEA and CA724 in the diagnosis of colorectal cancer has high sensitivity and accuracy. The three combined detection can complement each other and improve the diagnostic efficiency, which is of high clinical value for the diagnosis of colorectal cancer.

ABSTRACT

Objective To analyze the clinical significance of elevated serum carbohydrate antigen 724 (CA724) in gout patients. Methods The clinical data of 271 gout patients, 63 ankylosing spondylitis patients, 40 psoriatic arthritis patients, 67 osteoarthritis patients and 280 healthy controls in our hospital from Jul. 2016 to Jun. 2019 were retrospectively analyzed. The positive rates and levels of serum CA724 were compared among gout patients and ankylosing spondylitis patients, psoriatic arthritis patients, osteoarthritis patients and healthy controls. According to the clinical symptoms and signs, gout patients were also divided into acute-stage group (n=53) and chronic-stage group (n=218); the serum CA724 levels were compared between the two groups and the correlation between serum CA724 level with white blood cell count and C-reactive protein (CRP) was analyzed in acute-stage gout patients. According to the serum CA724 level, gout patients were divided into elevated serum CA724 group (n=63) and normal serum CA724 group (n=208), and the serum uric acid, triglyceride, and high-density lipoprotein were compared between the two groups. Results The positive rate of serum CA724 in gout patients was significantly higher than those in ankylosing spondylitis patients, psoriatic arthritis patients, osteoarthritis patients and healthy controls (P<0.05, P<0.01). Serum CA724 in gout patients was significantly higher than those in osteoarthritis patients and healthy controls (P<0.05, P<0.01). Serum CA724 in acute-stage gout patients was significantly higher than that in chronic-stage gout patients (P<0.01), and it was positively correlated with white blood cell count and CRP (both P<0.01). The levels of serum uric acid and triglyceride in gout patients with elevated serum CA724 were significantly higher than those in patients with normal serum CA724 (P<0.01, P<0.05), while the level of high-density lipoprotein was significantly lower than that in normal serum CA724 group (P<0.01). Conclusion Serum CA724 level is increased in some gout patients, and serum CA724 is correlated with acute gout attack and inflammation.

ABSTRACT

Objective To explore the diagnostic value of serum carcinoembryonic antigen (CEA), carbohydrate antigen 724 (CA724) combined with long non-coding RNA MALAT1 in gastric cancer. Methods A total of 100 patients with gastric cancer diagnosed in Changzhou Cancer Hospital, Soochow University from March 2013 to February 2018 were collected as the case group, and 100 healthy people during the same period in our hospital were collected as the control group. The levels of serum CEA, CA724 were detected by using chemiluminesent immunoassay assay, and the relative expression of MALAT1 was detected by using real-time fluorescence quantification polymerase chain reaction (PCR). The relationships between CEA, CA724, MALAT1 and the clinicopathological features of gastric cancer were analyzed. Receiver operating characteristic (ROC) curve was used to analyze the diagnostic value of CEA, CA724 and MALAT1 in patients with gastric cancer. The odds ratio (OR) and 95% confidence interval (CI) were calculated by using logistic regression. Results The levels of serum CEA, CA724 and MALAT1 in the case group were higher than those in the control group [(3.08 ±0.21) ng/ml vs. (0.74 ±0.11) ng/ml, t= 3.814, P= 0.002; (4.28 ±0.17) U/ml vs. (0.51± 0.07) U/ml, t=4.703, P<0.01; (113.9±3.9) vs. (36.1±1.2), t=7.160, P<0.01 respectively], and the three indexes were related to the pathological types and TNM staging of gastric cancer (all P< 0.05), and the most significant increase was seen in tubular adenocarcinoma patients and stage Ⅲ-Ⅳ patients. ROC curve analysis showed that the area under curve (AUC) of serum CEA, CA724 and MALAT1 in the differential diagnosis between the gastric cancer and the healthy people were 0.675 (95%CI 0.578-0.755, P= 0.001), 0.865 (95%CI 0.800-0.922, P< 0.01) and 0.847 (95%CI 0.771-0.911, P< 0.01), and the sensitivity was 69.1%, 80.2%, 85.8%, and specificity was 63.5%, 77.4%, 74.5%, respectively. AUC of combination of the three indicators for diagnosis of gastric cancer was 0.887 (95%CI 0.821-0.949, P<0.001), and the sensitivity was 84.3% and the specificity was 91.9%. Logistic regression results showed that high levels of serum CEA, CA724 and MALAT1 were risk factors for gastric cancer. After adjusting for age, sex, smoking history and drinking history, high levels of serum CEA, CA724 and MALAT1 remained the independent risk factors for gastric cancer. Conclusion The combined detection of serum CEA, CA724 and MALAT1 can be used to predict the risk of gastric cancer, and the three have a value for the diagnosis of gastric cancer, which is worthy of clinical application.

ABSTRACT

Objective To explore the expressions and clinical values of pro-gastrin-releasing peptide(ProGRP) and carbohydrate antigen 72-4 (CA72-4) in patients with gastric cancer.Methods Ninety patients with gastric cancer and fifty healthy subjects were selected from January 2014 to December 2016 in our hospital.Serum levels of ProGRP and CA72-4 were detected by electrochemiluminescence.The relationships between ProGRP and clinicopathological characteristics,postoperative recurrence and CA72-4 were analyzed.The diagnostic values of ProGRP and CA72-4 in gastric cancer were analyzed by receiver operating characteristic (ROC) curve.Results The expressions of ProGRP and CA72-4 in patients with gastric cancer were (249.3 + 28.9) pg/ml and (148.8 + 33.5) U/ml respectively,which were significantly higher than those of healthy subjects [(14.4 ± 7.6) pg/ml and (3.8 ± 1.4) U/ml],and the differences were statistically sigificant (t =56.320,P < 0.001;t =30.504,P < 0.001).The expression of ProGRP in TNM stage Ⅲ-Ⅳ [(269.1 ±30.9)pg/ml] was obviously higher than that in stage Ⅰ-Ⅱ [(198.5 +23.9)pg/ml],with a significant difference (t =11.200,P < 0.001).The expression of ProGRP in patients with lymph node metastasis [(259.9 ±31.4)pg/ml] was significantly higher than that in patients without lymph node metastasis [(190.3 ±26.8)pg/ml],with a significant difference (t =9.500,P < 0.001).The expression of ProGRP in patients with postoperative recurrence after one year [(181.3 ± 21.7)pg/ml] was higer than that in patients without postoperative recurrence [(26.1 ± 12.8)pg/ml],with a significant difference (t =31.830,P < 0.001).There was a positive correlation between serum ProGRP and CA72-4 (r =0.792,P =0.012).According to the ROC curve,the cut-off point of ProGRP was 23.6 pg/ml,and the diagnostic sensitivity was 80.0%,the specificity was 70.0%.The cut-off point of CA72-4 was 11.2 U/ml,and the diagnostic sensitivity was 60.0%,the specificity was 89.0%.The sensitivity and specificity diagnostic value of combined detection were 89.7% and 94.8%,better than those of individual detection (x2 =6.028,P =0.009;x2 =4.675,P =0.031).Conclusion ProGRP and CA72-4 are highly expressed in the serum of gastric cancer patients,with a positive correlation.The combined detection of ProGRP and CA72-4 can improve the diagnostic sensitivity and specificity.ProGRP is significantly correlated with tumor stage,lymph node metastasis and prognosis,which may be a mew target for prevention and treatment of gastric cancer.

ABSTRACT

Objective To explore the change and clinical significance of the serum carcinoembryonic antigen (CEA)and carbohydrate antigen 724(CA724)in patients with rectal cancer before and after neoadjuvant ra-diotherapy and chemotherapy.Methods The serum levels of CEA and CA724 of 30 patients with rectal carci-noma were detected by electrochemiluminescence method and were compared with those in 30 healthy people. Results The serum levels of CEA and CA724 in rectal carcinoma patients before neoadjuvant radiotherapy and chemotherapy were significantly higher than those in the healthy people,the difference was statistically significant(P<0.05).The serum levels in complete remission and partial remission patients after eight-week neoadjuvant radiotherapy and chemotherapy were significantly lower than those before treatment,the differ-ence was statistically significant(P<0.05),and the reduction levels were obviously higher than those in the stable group,the difference was statistically significant(P< 0.05).The CEA and CA724 levels in the stable treatment patients were also lower than those of before neoadjuvant radiotherapy and chemotherapy,the difference was statistically significant(P<0.05).The serum levels in the progression group were higher than those of before treatment(P<0.05).The serum levels of CEA and CA724 before neoadjuvant radiotherapy and chemotherapy existed a positive correlation(r=0.862,P=0.000).Conclusion The serum levels of CEA and CA724 in rectal cancer are highly expressed,suggesting that both of them are closely related to the occur-rence and development of rectal carcinoma,the measurement of the serum level changes of CEA and CA724 in patients with rectal cancer before and after treatment contributes to estimate the efficacy of neoadjuvant radio-therapy and chemotherapy and tumor progression.

ABSTRACT

Objective:To explore the predictive value of human serum epididymis protein 4 (HE4), carbohydrate antigen 72-4 (CA72-4) and vascular endothelial growth factor (VEGF) for the recurrent risk of gynecological tumor. Methods: A total of 142 patients with gynecologic malignant tumors were divided into the observation group, and all of them were followed up, and their recurrence rate within 2 years after operation was calculated. At the same time, 50 healthy women were divided into control group. The peripheral venous blood of all subjects were collected to test serum HE4 and VEGF levels by using enzyme-linked immunosorbent assay (ELISA). The serum CA72-4 was detected by using Roche electrochemiluminescence immunoassay. Three indicators of two groups were compared. Results: Before treatment, the serum HE4, CA72-4 and VEGF levels of the observation group were significantly higher than those of the control group (t=58.971,t=26.795,t=42.021;P<0.01). And the positive rate of HE4, CA72-4 and VEGF of the gynecological tumor group were significantly higher than those of the control group (x2=50.061, x2=37.596,x2=43.765,P<0.01). Within post-operative 2 years, there were 84 cases were recurrence in observation group. And the HE4, CA72-4 and VEGF of recurrent patients were significantly higher than that of non-recurrent patients (t=53.075,t=22.211,t=55.948,P<0.01).The sensitivity and negative predictive value of the combined detection of 3 indicators were significantly larger than that of single detection of CA72-4 (x2=8.537,x2=5.345, P<0.05).Conclusion: Patients with gynecological malignant tumor have high expression of serum HE4, CA72-4 and VEGF and the combined detection of 3 indexes possessed more sensitive, and it has greater predictive value for the recurrent risk of gynecologic malignant tumor.

ABSTRACT

Objective To explore the clinical application of DKK-1,TFF3 and CA72-4 detection in the diagnosis and treatment of gastric cancer.Methods Seventy-five cases(gastric cancer group) of gastric cancer admitted to our hospital from January 2013 to May 2015 were selected.Seventy cases of benign gastric disease(benign gastric disease group) and 70 persons undergoing the physical examination(healthy control group) were selected as the research subjects.The concentration of CA72-4 in each group was detected by the electrochemiluminescence analyzer.The serum DKK-1 and TFF3 levels in each group were detected by enzyme linked immunosorbent assay(ELISA).The receiver operating characteristic(ROC) curve was drawn for evaluating the diagnostic efficiency of each index in each group.The sensitivity,specificity,positive predictive rate and negative predictive rate of 3 indicators for diagnosing gastric cancer before operation were compared.The concentration change of various indexes after gastric cancer radical resection were compared..Results The concentrations of DKK-1,TFF3 and CA72-4 in the gastric cancer group were significantly higher than those in the benign gastric disease group and healthy control group,the difference was statistically significant(P<0.05).The area under ROC curve of DKK-1,TFF3 and CA72-4 were 0.876(95%CI 0.803-0.950),0.944 4(95%CI 0.894-0.975) and 0.818(95%CI 0.726-0.884) respectively.The sensitivity of CA72-4 was 78.6% and the specificity was 84.3%,the sensitivity of DKK-1 was 93.6% and the specificity was 87.1%,the sensitivity and specificity of TFF3 was 92.4% and 90.1% respectively.The concentrations of DKK-1,TFF3 and CA72-4 after radical resection in the gastric cancer group were significantly reduced,the difference was statistically significant compared with before treatment(P<0.05).Conclusion The detection of DKK-1,TFF3 and CA72-4 has a certain clinical value for the diagnosis of gastric cancer.The combined detection of these 3 indicators is conducive to improve the specificity and sensitivity of gastric cancer diagnosis.

ABSTRACT

Objective To characterize the clinical features of patients with high level of carbohydrate antigen (CA)724 in order to facilitate the recognition of this clinical manifestations.Methods Eight-two cases of hospitalized patients diagnosed with gout were selected and divided into 2 groups according to the level of CA724,and the clinical features and laboratory indexes were compared between the 2 groups.T test was used in the numerical variable data between groups,x2 test was used in the categorical variable data.Correlation analysis was tested using Spearman analysis.Results The incidence of gout stones in the group with high level of CA724 was 61%,which was significantly higher than that in the group of normal CA724 of 28%.There were statistically significant differences between the two groups (x2=8.894,P<0.05).The incidence of kidney stones in the group with high level of CA724 was 65%,which was significantly higher than that in the group of normal CA724 of 22%.There were statistically significant differences between the two groups (x2=15.013,P<0.05).But there were no statistically significant differences in age of onset,blood uric acid,erythrocyte sedimentation rate,and serum creatinine between the two groups (P>0.05).Conclusion The significantly elevated level of CA724 canappeare in gout patients,But it isnotcorrelated with tumor,which should be paid attention by clinician.It helps to avoid unnecessary checks and reduce the psychological and financial burden of patients.

ABSTRACT

Objective To explore the influence of different doses and courses of colchicine on tumor markers in patients with gouty arthritis.Methods Totally 102 patients with acute gouty arthritis were divided into group 1 (had taken colchicine before in hospital)and group 2 (hadn't taken colchicine before in hospital).Group 2 was divided into two subgroups,group 2A and group 2B.We also included 31 cases of non-gouty arthritis patients in the same period in the hospital as control group.Group 2A and group 1 were given 0.5 mg bid of colchicine and corresponding NSAID on the day admitted to hospital.Group 2B were given the same dose of colchicine the next day.Blood sedimentation,c-reactive protein,serum creatinine,blood uric acid,CA724,AFP,CEA,CA125,CA153 and CA199 were detected in all patients on the next day admitted to hospital.Then we analyzed the differences and influencing factors among the groups.Retest CA724 1 week later,and follow-up parts of patients,reevaluate CA724 again after stopping using colchicine.Results Compared with the control group,CA724 significantly elevated in patients who took colchicine,and the difference was statistically significant.CA724 level of day 1 in group 2A was obviously higher than that of group 2B,and the difference between the two groups was statistically significant.It showed that very low dose colchicine can also result in higher CA724,and the level was closely relative to total dose and using course.After stopping drug,CA724 value can drop to normal range.Conclusion Colchicine can significantly affect the level of serum CA724,which gradually rise with the increase of drug dose and course.It can be back to normal after drug withdrawal.The discovery can avoid the clinical misdiagnosis and excessive medical treatment,It has certain guiding significance.

ABSTRACT

Objective To evaluate the clinical value of the combined detection of serum human epididymis protein 4(HE4) ,carbo‐hydrate antigen125(CA125) ,carbohydrate antigen199(CA199) and carbohydrate antigen724(CA724) in the early diagnosis of ovar‐ian cancer .Methods 40 cases of ovarian cancer verified by pathological examination (ovarian cancer group) ,40 cases of ovarian be‐nign tumor (ovarian benign tumor group) and 40 individuals undergoing the physical examination(healthy control group) were se‐lected .The levels of CA125 ,CA199 and CA724 were measured by the electrochemiluminescence method ,the HE4 level was meas‐ured by the enzyme‐linked immunosorbent assay(ELISA) .The values of single index detection and the combined detection in diag ‐nosing ovarian cancer were analyzed .Results Serum HE4 ,CA125 ,CA199 and CA724 levels and positive rates in the ovarian cancer group were significantly higher than those in the ovarian benign tumor group and the healthy control group (P 0 .05) .The positive rate of the combined detection was 92 .5% ,which was significantly higher than the single index detection (P < 0 .05) .In the comparison of HE4 ,CA125 ,CA199 and CA724 single detection ,the sensitivity and specificity of HE4 detection were best .The detection rates of the stage Ⅰ and Ⅱ of ovarian cancer in the combined detection were significantly higher than that in the single index detection (P<0 .05) .Conclusion The combined detection of serum HE4 ,CA125 ,CA199 and CA724 might increase the detection rate of early di‐agnosis of ovarian cancer .

ABSTRACT

Objective To establish a simple and practical superparamagnetic immunochromatographic test strip for rapidly monitoring human serum level of CA72-4. Methods Water-soluble carboxylated super-paramagnetic nanoparticles were prepared with a modified one-step hydrothermal synthesis method. Magnetic probe was prepared by immobilizing specific antibody (mAb1) onto the surface of nanoparticles. Following with optimization and assembly of the test strip , we evaluated sensitivity , specificity , stability of this method for serum CA72-4 detection. Results The optimized test strip provided not only the qualitative results, but also the high sensitivity quantitative detection through stable magnetic signal. The detection limit was 0.83 IU/mL. One hundred clinical samples ( 70 positive and 30 negative ) were measured to assess these test strips with high sensitivity (99%) and high specificity (93%). The test strip and magnetic signal possessed high stability. Conclusion A rapid and quantitative detection of CA72-4 by the test strip was accomplished. This method is rapid, sensitive and quantitative, possessing great potential in large sample screening or in-home testing.

ABSTRACT

Objective To investigate the correlation of Helicobacter pylori (HP) infection and serum carbohydrate antigen 724 (CA724) level .Methods 229 patients with HP infection hospitalized in gastroenterology department were enrolled in the observa‐tion group ,and patients were divided into different disease group according to the results of diagnosis .Other 100 healthy people , conducted physical examination ,were enrolled in the healthy control group .The HP infection was detected by 13C‐urea breath test and the serum CA724 level was detected by chemiluminescent immunoassay .Results The HP value and serum CA724 levels in each disease group were higher than those in the healthy control group ,with significant differences(P0 .05) ,except the duodenal group .The serum CA724 level was highest in the gastric cancer group ,there was no statistical differences between each disease groups(P>0 .05) ,except the gastric cancer group .The serum CA724 level was positively correlated with HP value in patients with HP infection(r=0 .331 ,P<0 .01) .Conclusion HP infection may correlate with serum CA724 level .The serum CA724 level in the gastric cancer group increased more significantly ,in other disease groups increased steadily .Dynamic observation of serum CA724 level in patients with upper gastrointestinal tract disease associated with HP infection could be helpful for early diagnosis and pre‐vention of aggravation of lesions ,and could provide effective diagnosis basis for early detection of upper gastrointestinal tract le‐sions ,such as gastric cancer .

Objective To evaluate the diagnostic value of the level of plasma D-dimer,high-density lipoprotein (HDL),carcino embryonic antigen (CEA) and carbohydrate antigen 724 (CA724) in gastric cancer.Methods The plasma and clinicopathological data of 103 gastric cancer patients and 111 normal controls were collected.The levels of D-dimer,HDL,CEA and CA724 were detected.SPSS 13.0 statistical software was applied to analysis the sensitivity and specificity of each examination method and to find out the appropriate combination.Results The levels of D-dimer,CEA and CA724 in patients with gastric carcinoma were 0.87 (2.69) μg/ml,2.66 (4.38) ng/ml,5.10 (7.79) U/ml,respectively,they were distinctly higher than those in normal controls [0.22 (0.21) μg/ml,1.28 (1.60) ng/ml,1.81 (1.60) U/ml,all P =0.000].HDL level was significantly lower in patients than that in normal controls [0.86 (0.35) mmol/L vs 1.29 (0.44) mmol/L,P=0.000].The area ofROC curve of D-dimer,HDL,CEA,CA724 were 0.799,0.859,0.739,0.743,respectively.The cut-off of D-dimer was 0.46 μ.g/ml,the sensitivity was 68.0 %,the specificity was 86.5 %.The cut-off of HDL was 0.995 mmol/L,the sensitivity was 73.8 %,the specificity was 84.7 %.The cut-off of CEA was 3.585 ng/ml,the sensitivity was 44.7 %,the specificity was 92.0 %.The cut-off of CA724 was 3.765 U/ml,the sensitivity was 57.3 %,the specificity was 89.2 %.The sensitivity of D-dimer+HDL+CA724 was 83.5 %,the specificity was 89.2 %.The sensitivity and specificity of D-dimer+HDL+CEA+CA724 were 84.5 % and 89.2 %,respectively.Conclusions The D-dimer+HDL+CEA+CA724 may provide the evidence for diagnosis of gastric cancer.Combined detection has higher sensitivity and specificity.

ABSTRACT

Objective To study the value of serum HMGB1(high mobility group box-1) and carbohydrate antigen 72-4(CA72-4) in patients with early gastric cancer .Methods 50 cases of early gastric cancer patients were enrolled as a group ,45 cases of gastric lesions as benign gastric benign group ,100 cases of healthy people as healthy group.Serum HMGB1 and CA72-4 in all people were detected .Results Early gastric cancer serum HMGB1 and CA72-4 was higher than benign lesions in the stomach and the healthy group (χ2 =33.69,82.95,51.41,104.74, P0.05).HMGB1 diagnosis of early gastric cancer sensitivity was 70.0%,specific-ity 95.2%and accuracy was 88.7%.CA72-4 diagnosis of early gastric cancer with a sensitivity of 80.0%(40/50), specificity was 97.2%(141/145) an accuracy of 92.8%(181/195).Joint detection,diagnosis of early gastric cancer with a sensitivity of 94.0%(47/50),while the specificity was 93.1%(135/145),the accuracy was 93.3%(182/195),higher than those of the single detection sensitivity and accuracy .Early gastric cancer with lymph node metasta-sis in patients with serum HMGB1 and CA72-4 were significantly higher than those in patients without lymph node metastasis,the two groups were significantly different (t=2.927,4.096,all P<0.05).Conclusion Joint detection of serum HMGB1 and CA72-4 allows early diagnosis of gastric cancer ,and to determine the prognosis of patients .
